PHỤ HUYNH KÝ NHẬN – PARENT’S ACKNOWLEDGEMENT & AGREEMENT
Tôi, phụ huynh của học sinh ghi danh ở trên, cho phép thầy cô đưa con em chúng tôi đi nhà thương hay bác sĩ trong trường hợp khẩn cấp, cùng ký nhận đã đọc và chấp nhận các điểm kỷ luật của Ban Giáo Lý & Việt Ngữ. Chúng tôi sẵn sàng thông cảm và sẽ không kiện các thầy cô, nhà trường, Giáo Xứ vì những tai nạn rủi ro có thể xảy đến cho con em chúng tôi trong giờ các thầy cô tình nguyện giảng dạy Giáo Lý & Việt Ngữ, lo sinh hoạt cho trẻ.
In case of emergency, I, the parent of the above-mentioned child, give the permission to any hospital or to any physician selected by the chaperon to render medical treatment deemed necessary and appropriate. Any resulting hospital, medical or related costs and expenses will first be paid by the medical insurance or benefit plan of my spouse or mine. I also read and agreed with the Ban Giáo Lý Việt Ngữ program policy. I, on behalf of myself, my heirs, assigns, executors and personal representatives, release, hold harmless and discharge forever the Roman Catholic Church, the Diocese of San Jose, Our Lady of La Vang Parish, its staffs and its officers, leaders, catechists, volunteers and affiliates from any and all liability, claim, loss, damage, cost or expense and waive any claims against any such person or organization arising directly or indirectly from and attributable in any legal way to any action or omission to act of any such person or organization in connection with the sponsorship, organization and services relating to the program.
